From my experience something has changed. Either a permission on a folder or a path to the image or if you used variables in your template you made a change to one of them.If this is not a new computer...

We have also used the pxe boot environment to remote control a machine to fix it if it had a corrupted file or etc. Now what is not usable either.

That was always broken in our env. We use full drive encryption which will break a PXE Boot Task run against the device

I just verified that scheduling a provisioning task doesn't work either just like Laszlo indicated. I have to say 9.5 in general has been a disappointment.

I haven't applied any patches to the core. I am pretty sure I have updated the pxe rep to the 4 version, but I am willing to check it to make certain. Where would I look to know for sure?

When you say latest, which patch specifically are you using? If you have base 0228 there is a known issue with software distribution in provisioning. Make sure you are using base 0228A.
