Yes you are correct that at minimum you only need the following for WOL to work with the CSVIMPORT:

 

Device Name = %1% Network - NIC Address = %2% Network - TCPIP - Subnet Mask = 255.255.255.0 Network – TCPIP - Address = %3% Network – TCPIP – Subnet Broadcast Address = %4%

 

If this isn't working and you powered the machine on already before in the same subnet then either we have the wrong MAC Address or the magic packets are not being configured properly from the core to point to the proper machine. If you run Wireshark on the core do you see the magic packet leave with the proper MAC Address? If we statically elect a Multicast Domain Rep on that subnet and select the Wake on WAN option does that allow it to work properly?

We may even want to run wireshark (in promiscuous mode) on a machine in that same subnet to validate the receipt of that packet on the subnet.
